Waivers

Waiver Form

Every orienteering participant (including adults accompanying junior orienteers) must hold a $5 YOA membership and submit a signed Orienteering Canada waiver (adult) or assumption of risk form (minor). One form is required per person. You may complete this form electronically and email to registration(at)yukonorienteering.ca, or you may drop it off at your first event.

Health Declaration

For 2021, an Orienteering Canada health declaration must be completed and submitted at each in-person event or training session. Printing and filling out the form out ahead of time, will save time at the event. A separate form is required for each participant.
